Clogged milk ducts are a common issue for postpartum breastfeeding individuals, affecting up to 20% during the early stages of breastfeeding. These blockages happen due to inflammation around the milk duct, often caused by factors like infrequent feeding or pumping, improper latch techniques, tight clothing, or stress. Hyperlactation can also contribute. If untreated, inflammation can lead to discomfort and possibly mastitis. Promptly addressing clogged ducts is crucial to relieve pain and ensure continued effective lactation.

Your First Visit: The Initial Assessment

Your first session starts with a thorough. We’ll talk about your medical and breastfeeding history, when your symptoms began, and what you’ve tried so far. Your therapist may gently assess the breast tissue (only if appropriate and with your full consent) to identify any areas of tightness, swelling, or tenderness.

Because tension and posture can play a big role in how milk flows, we’ll also look at your chest, shoulders, and rib mobility, as well as how you typically feed or pump. Every step is explained by your therapist, so you always feel in control and comfortable.

Developing Your Personalized and effective Mastitis and Clogged Duct Treatment Plan for best results

No two cases of mastitis or clogged ducts are the same. Based on what we find, your therapist will create a plan designed to relieve symptoms quickly and prevent them from coming back.

This may include:

  • Gentle manual therapy and lymphatic drainage to improve milk flow and reduce inflammation

  • Therapeutic ultrasound to ease discomfort and support tissue healing

  • Postural and breathing work to take pressure off sensitive areas

  • Collaboration with your lactation consultant or OB-GYN if needed

You’ll leave with clear steps to support healing at home. Your therapist may teach you:

  • How to perform gentle self-massage

  • When and how to use cold therapy to calm inflammation

  • The best feeding or pumping positions to reduce compression

  • What clothing, movment, or support to avoid while healing

  • What to watch for and when to reach out if symptoms change

Our approach focuses on giving you control and confidence in your recovery, not just temporary relief. You’ll understand different strategies and when to use them so that you’re not reliant on us.

Treatment Sessions needed

Many patients notice improvement after just one session, with significant relief within 1–3 visits when symptoms are caught early.

Some may benefit from additional sessions if there are underlying posture or muscle-tension contributors.

Your therapist will always be transparent about what’s needed and what isn’t and will work with you to develop a plan that works to get maximal results.

Questions You May Have About Mastitis and Clogged Duct Treatment

  • Mastitis often feels more painful, with redness, swelling, and sometimes flu-like symptoms. A clogged duct usually presents as a firm, tender lump without fever. If you’re unsure, our physical therapists can help assess and guide next steps.

  • Yes. In many cases, conservative care like gentle manual therapy, ultrasound, and lymphatic drainage can relieve symptoms before infection develops. If infection is suspected, we coordinate with your healthcare provider to combine treatments safely.

  • No, techniques are gentle and designed to relieve pressure and improve flow, not cause discomfort. Many patients feel noticeable relief after just one session.

  • Early intervention (within 24–48 hours of symptom onset) can prevent progression and make recovery much faster. But, if it’s been a while since you started having symptoms and are still having pain and/or are concerned, we’d still love to help you.

  • Absolutely. In fact, frequent nursing or pumping is encouraged to help clear the affected area. Your therapist can help you find comfortable positions to feed or pump.

  • Recurring mastitis can result from unresolved tightness, posture, or feeding patterns. Physical therapy addresses these root causes to help prevent future episodes.

  • Therapists may use gentle therapeutic ultrasound (to support milk let-down and reduce swelling), lymphatic massage, neural mobilization (to free up nerves around the ribs and chest), and kinesiology taping to help reduce inflammation and encourage drainage.

  • No. Deep or aggressive breast massage can worsen inflammation and pain. The goal is gentle, targeted techniques. The Southern Pelvic Health protocol explicitly cautions against painful, deep massage when treating clogged ducts.

  • Most patients experience effective relief in just 1–2 visits, though this depends on severity, timing, and individual factors.

  • Yes. Many of the same techniques can be used alongside medical care (e.g., antibiotics) for mastitis. Your physical therapist can coordinate with your healthcare provider to safely integrate hands-on care, education, and self-management.

  • Therapists can help with posture and feeding ergonomics (how to hold baby, positioning), self-massage (light, directional), proper feeding or pumping schedule, decompressing clothing, and possibly taping strategies to support drainage.

  • Absolutely. Musculoskeletal factors (tight muscles, restricted nerve mobility, posture strain) can contribute to how milk moves, tissue stress, or localized pressure. Part of PT’s role is to assess and address these underlying contributors.

  • If symptoms persist beyond a few days, it's a signal to get professional help (lactation consultant, physician, or physical therapist). Persistent blockages may indicate a developing infection or require hands-on intervention.
